For this series am putting it as Trivia as I can't find the changes between the novel and anime, there also a manga counterpart to Index so for this series there is going to be multiple thread. This thread will not only contain Trivia but also the changes between three different version for Index and two different versions of Railgun as well as Accelerator.
Update: Do note the changes will be written under Note which will be at the end of every arc, also Railgun has some episodes that are original hence I will state if the episode is original or not. Added the whole of Index Season 1 plus 1 episode from Season 2 as that episode is link with that arc which I had stated clearly. Railgun Season 1 to be added tomorrow along with Index Season 2 if there is no delay that is.
Update 2: Added Railgun Season 1 and Index Season 2 as promised, I will added Railgun Season 2 and Index Season 3 three to four weeks from now which could be second week or third week of April. On the side note Accelerator Season 1 will air on July, hence the Trivia should be up once the series ends as it would be easier for to comply the information and post it in one shot instead of update it on weekly base or monthly base.

Episode 47 - Dawn of a Revolution (Silent Party) (Mainly Original Episode)

Episode 40 - Sisterhood

With the aid of Mikoto and the clones, Touma is able to defeat Accelerator.
The Level 6 Shift experiment is suspended, and the clones are sent to facilities for readjustment on their bodies.
Touma refers to Mikoto by her family name for the first time.
Trivia
With at least 15 episodes dedicated to the story, the Toaru Kagaku no Railgun S version of the Sisters Arc is the longest story arc in both anime series.
Index makes a cameo appearance for the first time in Toaru Kagaku no Railgun S.
1 minute and 30 seconds of content are reused from the previous one as the cold open for this episode.
Scenes from episode 4 are used as a flashback for this episode.
Overall Trivia
With 21 chapters published, this arc is the second longest one in the Toaru Kagaku no Railgun manga. Moreover, with 16 episodes in the Toaru Kagaku no Railgun S anime adaptation, it is the longest story arc in the animated series.
This is the first arc in the Toaru Kagaku no Railgun sidestory to intersect with the main story of Toaru Majutsu no Index.
Note:
In all versions of the story, a blimps appear to foreshadow Mikoto's involvment, the scene that plays out the most vital is the scene where Mikoto declares in front of Touma that she despises blimps. In the original novel, the Mizuho Organization is shown in the blimp to have withdrawn, during Touma and Mikoto's first meeting after his memory lost. In the scene where she meets Touma a second time after his memory lost, the narration references that three muscular dystrophy research institutes have withdrawn. This is carried over in the anime adaptation. In the manga adaptation, the scene in their first meeting is removed, though it is later shown in their second meeting, however, the Duchenne Dystrophy Research Center is the one that is referenced. This segment is removed in its entirety in the Toaru Kagaku no Railgun manga, as the audience is aware of that Mikoto is involved; although, the blimp can still be seen in some panels. Their second meeting, also redacts the blimp reporting about the research institutes, and instead focuses on the subject around Tree Diagram.
This scene is slightly modified in the Toaru Majutsu no Index manga adaptation Chapter 12. Tsuchimikado Maika does not appear in the scene, and since the Deep Blood Arc was not adapted into the manga, Aisa is not present, leaving only Index present for the scene.
The scene plays out somewhat differently in the original novels, as Touma realizes that he is unable to escape both the boxes and the oncoming Accelerator, Touma takes advantage of Accelerator's reflection and attacks him with gravel, which automatically counters Touma, ejecting gravel upon him and propelling Touma far enough to escape the falling containers.
The original novel has Touma blown away by Accelerator's use of the wind, and is later knocked unconscious as his body is thrown into a wind turbine's pillar. The anime plays this scene differently from the rest of the adaptations, hither, Accelerator already begins forming the plasma ball and actually attacks Touma with it and knocking down. Only the 37th Chapter of the Toaru Kagaku no Railgun manga keeps this scene intact, while the Toaru Majutsu no Index manga keeps the wind attack but has Touma fall down into several shipping containers instead.
It is noted that the researchers could have used Last Order to prevent the Sisters from gathering and manipulating the wind turbines, but stayed their hands as they might cause an irreversible damage to the experiment.
Chapter 21 of the Toaru Majutsu no Index manga adaptation completey redid the scene. Hither, Misaka 10032 and Misaka Mikoto meet just outside of Touma's ward, wherein Misaka 10032 later returns to the room to have Touma touch her breast again after referring to the reading of his bioelectricity. Needless to say, Mikoto becomes flustered because of this and later takes Misaka 10032 away.fter
The note section contains information from both Index Side and Railgun Side and it the only arc that crossover to the other series. As the next arc which both series had took after Index Side of the story with a couple of scenes being the same. That which be in Railgun Season 3 which is set to release somewhere this year.
Episode 39 - Tōma Kamijō
Major Event
Misaka 10032 battles Accelerator as part of the Level 6 Shift experiment.
Touma interrupts the battle between Misaka 10032 and Accelerator. Touma meets Accelerator for the first time and does battle with him.
The switchyard is destroyed, and a noticeable sudden storm appears over Academy City due to the ensuing battle.
Trivia
Despite being the protagonist of Toaru Majutsu no Index, this marks the first time Touma's name is used as a title for an episode.
Touma's phone is changed back to the regular fliphone he uses.
The Imagine Breaker sound effect adds a new "glass breaking" sound after the initial high-pitch sound indicative of Touma negating something is played.
Scenes from episode episode 5, episode 6, episode 12, episode 13, and episode 14 are reused for flashback sequences during this episode.
After the dust explosion that Accelerator initiated during the fight, fires begin erupting in the switchyard. After regaining consciousness after being knocked out briefly by Touma's punch, the fires are completely gone. This may indicate that the fire has snuffed itself out after quickly consuming all the oxygen it could in the windless night.
The second time Touma punches Accelerator resembles his Finishing Move animation sequence featured in Toaru Majutsu no Index (PSP game) barring the dragon.
Episode 36 - Tree Diagram
Major Event
Misaka 10031 and 10032 learn about experiencing the following emotions which are from then on permanently stored in the Radio Noise network: happiness, anger, frustration, irony, bewilderment, and sadness.
The 10031st Level 6 Shift experiment begins.
Mikoto discovers that Tree Diagram has already been destroyed.

Grow Slowly returns as the ending theme, replacing stand still from the previous episode.
Misaka 10032 identifies herself as such only after seeing both Misaka 10031 and Accelerator together for the experiment. Here, the Misaka 10031–10032 confusion is finally cleared during this episode as Misaka 10032 meets up with Misaka 10031, a fact that is only slightly shown in the manga and revelead in an omake of the 6th Toaru Kagaku no Railgun Manga Volume.
Touma references both Index and Himegami Aisa in her conversation with Misaka 10032 regarding taking care of cats, specifically Sphynx.
The Destruction of Tree Diagram and its Remnant is referenced during this episode.
The anime makes it clear that Mikoto was planning to stop the Level 6 Shift experiment by having Tree Diagram explain that the fight between her and Accelerator permanently prevented Accelerator from becoming a level 6 esper. She then planned to destroy Tree Diagram.

Episode 34 - Meltdowner
Major Event
The first actual single battle between Level 5s occur between Misaka Mikoto and Mugino Shizuri. Misaka Mikoto defeats Mugino Shizuri.
Misaka Mikoto successfully cripples the second to last laboratory connected to the Level 6 Shift experiments, and later discovers that the experiment can no longer be sustained.
Trivia
This episode marks the first time two Level 5s do battle in single combat in an anime adapatation. This is unlike Mikoto and Accelerator's confrontation, where the latter didn't even get the opportunity to fight former.
Touma does the preview narration for the next episode, the first time he does so in Toaru Kagaku no Railgun.
Scenes from episode 5, 6, and 7 are reused as flashbacks in the episode.
An animation error occurs when Mikoto tries to dodge Mugino's Meltdowner beams after finding out that she is somehow still conscious. Mikoto's head is cut-off, indicating that the frames were not aligned properly.
The animation used for Touma giving his money to the vending machine is exactly the same one from the 10th episode of Toaru Majutsu no Index.
Episode 32 - ITEM

Ami Koshimizu retains her role as Mugino Shizuri. She first appeared in the Toaru Majutsu no Index (PSP game).
Frenda stating that she nearly lost her pretty legs might be a reference to her terrible fate in the 15th volume of Index, where she is severed in her torso and killed by Mugino Shizuri after betraying ITEM to SCHOOL.

Episode 30 - I... Can See All of You

Misaka Mikoto fights Accelerator and is soundly beaten. She would have been eliminated by Accelerator if not for the timely efforts of the Sisters.

Misaka 8912 replaces Misaka 9982 as the first Sister to have tasted milk tea. Moreover, milk tea is a recurring theme between the Sisters, appearing in the ending sequence as well as the tea Misaka 9982 references when she is taken by Mikoto for a drink.
Several scenes from the previous episode are reused as a flashback, specifically, that of Misaka 9982.
The hospital flasback scene from episode 2 where Mikoto allows the scientist to use her DNA Map is reused for the nightmare sequence during this episode.

The episode marks the first time a pantyshot actually appears on the Railgun adaptation.
The scene where Misaka 9982 tries to save a baby from heatstroke completely removes Yomikawa Aiho's appearance.
Misaka 9982 references the tea scene from the 20th chapter of Railgun despite the scene being absent in the anime.
Misaka 9982 points out that the Japanese word for "young cat" (こねこ koneko?) is a palindrome. In one fan translation, the English palindrome "Was it a cat I saw?" is used.
Endymion, along with Misawa Cram School can be seen from the distance in the bridge scene where Misaka 9982 says farewell to Mikoto. Moreover, this is the same bridge used when Kamijou Touma and Stiyl Magnus used to travel towards Misawa Cram School back in episode 7 of Toaru Majutsu no Index as well as the bridge used at the end of the first opening of Toaru Majutsu no Index II.
In the code, each group of three letters happen to be the first three letters on each line of a keyboard.
Furthermore, in the code each group of numbers happens to match up with the buttons of an atm machine going from bottom to top and left to right.
Scenes of Misaka 9982 has her goggles drawn traditionally or of poorer quality.
Compared to his original appearance in the Toaru Majutsu no Index anime, Accelerator has noticeably longer and wavier hair in his apperance here.
When Misaka 9982 fires her Toy Soldier, the scene is computer generated, moreover, there is no rifling marks on the bullets.
Despite the experiment being referred to by Misaka 9982 as the 9982nd experiment, the file that Mikoto reads regarding the experiment has it written as Misaka 9928.
While Misaka 9982 crawls toward the Gekota badge after her legged ripped off, a scene where her entire body is shown along with her missing leg. However, the scene is censored in the television airing, with a black bar diagonally placed on screen to cover her leg.
The ending theme is cut during this episode, an unprecedented move in the animated Toaru Majutsu no Index series. Moreover, the entire credits sequence is changed into a scrolling one in a black background.
Episode 28 - Sisters

Misaka Mikoto meets a clone, Misaka 9982, for the first time and confirms her worst fears.
At the very end of the episode, after the ending sequence, Misaka 9982 is asked by the researchers to clean a blood-stained room full of dead clones who had fought in a test experiment against each other.

Overall Trivia
With only 1 and half episodes dedicated to covering the event (as the other half was dedicated with still covering the epilogue of the invasion), the Skill-Out Uprising, or more specifically the Toaru Majutsu no Index Light Novel Volume SS has the shortest adaptation of an event in the anime.
There also one chapter that was not feature in the anime and as of Season 3 it was announced that the missing chapter that was not featured in the anime will get a CD Drama.
The last point in Overall Trivia was added in by myself as I heard that they had plans to release it as CD Drama but the site that I got this information did not state on their site or at least I don't remember seeing it.

This marks the first time that an "Awakening" is shown, though no information is provided about it.
When Index intonated her song Accelerator moved his lips in sync, though it was to pronounce Kihara name. This may be a reference to the the event of World War III, when he truly sing a similar song.
When the scene focuses on Vento of the Front during her correlation of Fuse KAZAKIRI to her bleeding, if the viewer watches carefully the viewer can see that the background has blocks of pixels as if poorly enlarged.
There is a certain delay in Amata's reaction to Index moving closer to the table where Last Order is.
Index can be clearly seen to have already arrived at the table Last Order is placed in, however, in the next scene after Amata's reaction, she is now farther away from Last Order.

There are more than 10 Misaka clone in Academy City, furthermore, during the activation of the virus that would make Fuse=KAZAKIRI manifests, Misaka 10032 is the only clone apparently able to move and speak.
When Accelerator launches his attack on the Windowless Building, the building he launches at it simply bounces off sustaining little damage before falling back and then collapsing. A building travelling at that speed should have completely shattered upon hitting the target.
This episode would mark a new animation for Aleister Crowley's appearance.

This episode marks the debut of Hound Dog.
The scenes where the Accelerator fights the Hound Dogs are within the bloodiest moments of the novels, in the anime he easily satisfy himself with just beating them up.
During Accelerator's attempt to destroy Amata's gloves, the entire scene is filtered with the color blue, at the same time as this it takes several seconds for Accelerator to touch Amata's gloves while Amata was able to talk, move around Accelerator and deliver a punch to him in mere seconds.
Just before Touma notices one of the Anti-Skill fall unconscious, the Anti-Skill member had comrades next to him, however, when Touma came close to the unconscious Anti-Skill member, none of the other Anti-Skill next to him appears.
Vento does not push the transmit button on the hand-held radio she is using at any point during her conversation with Aleister, even though the radio is a type that can only transmit when the transmit button (The large, black, oval button on the left side of the radio) is pressed. It is possible, however, that Aleister was using some type of listening device or magic to listen in on the radio even though it wasn't transmitting.

Episode 36 - The Observatory (Belvedere)
Major Event
Lidvia Lorenzetti and Oriana Thomson begin their final preparations for the activation of St. Peter's Cross.
Kamijou Touma, Tsuchimikado Motoharu and Stiyl Magnus discover how St. Peter's Cross is activated, and continues their pursuit of Oriana Thomson, with Motoharu getting injured by Oriana in the process.
Kamijou Touma and Stiyl Magnus engage in combat against Oriana Thomson.
Trivia
In the Novels, Tsuchimikado Motoharu was taken out of the fight against Oriana by his hands and legs being burned by the fence, not being blasted by electricity. Other differences include the mention of the 'Kami-yan disease', and Misuzu making fun of Mikoto's relationship with Touma being omitted.
Motoharu called General Superintendent Aleister on his cellphone to help them infiltrate School District 23, but this episode did not mention Aleister's involvement when it was mentioned in the novels.
When Touma takes out the brochure to tell Motoharu that the bus Oriana took moves around district 7, right after the scene uses another bus to cut to a close up of Touma, the brochure disappears completely as well as Touma's posture changes.
The scenery that was shown in the ending song, Magic∞World where Touma, Index and Mikoto cross paths after walking is featured in this episode.
The large aeroplane featured during this episode seemed to dive down towards the ground, however, a scene immediately after shows that the aeroplane is ascending upwards.

Touma actually refers to Mikoto by name in this episode rather than by saying "you" or "Biri Biri."

Shirai Kuroko is seen on a wheelchair, despite being able to stand perfectly without trouble in the previous episode.
Though accidentally, Index is the first girl to have ever kissed Touma.
Though both members of Necessarius, as well as Stiyl referring to Motoharu back in the 2nd episode, this episode marks the first time both Stiyl Magnus and Tsuchimikado Motoharu appear onscreen.
When Laura holds up the Stab Sword to show it to Aleister, several scenes show that the handles of the sword is either touching Laura's hair or missing them all entirely.
The episode marks the first onscreen appearance of the "Spiky-haired girl with glasses" and "Girl with the headband" in the Toaru Majutsu no Index anime adaptation. Originally appearing in the background of some of the Light Novel artwork, they later appear in the manga adaptation as well.
When Aisa and Maika where in the same scene, Maika has noticeably thinner lines than Aisa, which may be due to her being rendered in CG.

Episode 31 - Coordinate Relocation (Move Point)

When Kuroko tried tried to hit Awaki with the lamp, she runs for a few seconds towards her. However, it is shown that there is only a short distance between them, and there is no need for Kuroko to actually run as there is no room.
Overall Trivia
This marks the first story arc where nearly the entire narration is not in Touma's perspective, the first arc that featured this was with Accelerator in the 5th Light Novel Volume story arc where he shares the narration with Touma. Accelerator would not get an entire story arc centered around his perspective nearly entirely until the 15th Light Novel's story arc.
Episode 24 - Dear My Friends (Original Episode)

With the aid of her friends, Mikoto is able to defeat Therestina who is later taken into custody.
With the use of the First Sample, Kiyama Harumi is able to revive her students and meet them for the first time since being comatosed.
Edasaki Banri meets Haruue Erii again since moving out of their Child Error facility.
Harumi's students meet Mikoto, Kuroko, Uiharu and Kuroko.
The Poltergeist earthquakes happening all around the city ends.
Trivia
The previous opening song, only my railgun, is used as an insert song during Mikoto's battle with Therestina in the highway.
The episode is named after Dear My Friend -Mada Minu Mirai he-, the previous ending theme for the anime. It is used as an insert song during the epilogue of the anime.
Therestina refers Academy City as an experiment and students as guinea pigs, which is technically correct if one has been keeping up with Aleister Crowley's plans in the light novels.
In the prologue, we see that Mikoto, Kuroko, Uiharu and Saten, in that order - are running towards the hospital the Child Errors were in. During that sequence we can see the background that was used in the promotional image for the anime; the background is also wraparound, as such the background repeats while they are running.
Several frames where Konori Mii was wearing her helmet, had her, either having her hair coming out of her helmet or none at all.
The scene where Mikoto falls to the ground after destroying one of the mechanical mobile suit's clamps, as well Kuroko teleporting after Mikoto summoned her, is a recycled animation from the opening, LEVEL 5-judgelight-.

Overall Trivia
It is unclear whether or not the Poltergeist Arc should be considered canon. In favor of its inclusion is the existence of Haruue Erii and Therestina Kihara Lifeline elsewhere in the Index universe.
Haruue Erii appears in a picture on Misaka Mikoto's phone in the Toaru Kagaku no Railgun manga.
Haruue Erii also appears in a colored illustration in the first Shinyaku Toaru Majutsu no Index novel.
Therestina Kihara Lifeline and her schemes (regarding the Ability Body Crystal) during the arc are refered to in the 22nd Toaru Majutsu no Index novel.
Kihara Enshuu mentions Therestina Kihara Lifeline by name in the 4th Shinyaku Toaru Majutsu no Index novel.
In favor of its exclusion is the presence of Kongou Mitsuko and the continued absence of Kiyama Harumi and her students.
Kongou Mitsuko does not tranfer to Academy City until about August 27th in the manga, and would not have been present before August 10th when the Poltergeist Arc takes place.
Kiyama Harumi does not appear again in either the Index novels or Railgun manga after her defeat and arrest in the Level Upper Arc.
But whatever the canonical status of the Poltergeist Arc may be, the Railgun manga and anime do not appear to be consistent regarding presence (or absence) of Kongou Mitsuko in Academy City before August 10th.

The appearance of Himegami Aisa is not to be confused with her first appearance in Episode 7 of Toaru Majutsu no Index because she already wears the cross necklace that protects her from her own power; note that she receives this after the Ars Magna arc. (However, Aisa's statement about being enrolled in Kirigaoka Girls' Academy and the appearance of Haruue Erii, coupled with the tremor in Erii's scene, contradict this info, as the source of the tremor is addressed on August 8, the exact day the Ars Magna arc takes place- thus, before Aisa acquires the cross necklace and is subsequently expelled from Kirigaoka Girls' Academy.)

Episode 9 - Majority Report

Trivia
Due to the fact that the previous episode removed Mikoto and Touma's meeting before he meets Index, this episode is forced to remove their meeting after Touma leaves after his remedial classes, as well as his recent meeting with Index in the morning. This would imply that summer vacation has not even started yet, and that their meeting is pushed back for much later in the altered anime timeline.
Despite Saten telling her mother that Academy City will not tamper with her head, one does not become an esper without the brain being altered in some form or other, using hypnosis, drugs, etc.
Despite Saten not believing that charms such as the one the one given to her by her mother could work. The existence of magic and the principles of Idol Theory however, allows the charm to work, though in a limited way.
This marks the first time Kuroko attempts to teleport an object directly into someone's body. This would not happen again until episode 7 of Toaru Majutsu no Index II, against fellow teleporter, Musujime Awaki.
The animation of Saten's running in the street after seeing the gang beat up Kikuhiko is reused after Kuroko's victory over Trick.
A still of Trick's face that is seen during his kick successful kick on Kuroko, is reused immediately after Kuroko lands inside the abandoned building.

Assuming that the Railgun and Index anime adaptations are of the same timeline, we can presume that Mikoto would meet Touma again much later that would kick off the story of Toaru Majutsu no Index. Meaning, Mikoto would again, cause another power outage in the city.

Episode 3 - Tokiwadai is Targeted (Original Episode)

The episode shows for the first time, the weather predicting powers of Tree Diagram, the sattelite that would be destroyed in the near future by Index, as show in episode 6 of the Index anime.
Despite Tokiwadai Middle School being featured in the past several times both in Index and in Railgun, this would be the first time that the surrounding premises, School Garden, is shown.
Despite Pasticceria Manicagni being Italian, everything else in the School Garden is French.
When Uiharu and the rest were checking out the Bank listing of esper powers, both Psychokinesis and Attention Power are listed, despite being both actually Telekinesis. Attention Power is the direct translation of Telekinesis' kanji (念動力).
Despite saying not noticing the pigeon, Kuroko is the first one to turn her head towards the window before Uiharu, though it could mean that Kuroko was only looking at another part of the sky where her gaze could not catch the pigeon.

Episode 1 - Electromaster

The vending machine, a recurring location in the Toaru Majutsu no Index universe, is given a name, referred as Vending Machine #7116.
This is the first time the System Scan is shown in the anime continuity.
Watanabe appears in this episode but will not be named until the 44th Chapter of the Railgun manga.
The scene where Kongou brags about her abilities is taken directly from the Prologue of 8th Index Light Novel Volume. Here, the scene is used to introduce Kongou as part of the cast, despite not appearing in the Railgun manga until the 40th chapter. The scene is also redone in the manga as well, as part of her introduction in the manga continuity.
In the episode, the shower scene after the System Scan is also based from the 8th Index Light Novel, and was also done at the last episode of the first season of Toaru Majutsu no Index anime adaptation, as well as the 6th episode of the second season. This has Kuroko in the anime continuity invade Mikoto's shower stall three times.
Saten is introduced earlier in the anime than in the manga, to complete the four girl ensemble theme going for the show. Her appearance in the manga regarding her opinion on Mikoto is different to that in the anime, which is that in the manga, she too shares Uiharu's awe of Mikoto.
Although her obsession with Gekota is downplayed in the first season of Index, it takes a prominent role in the anime, which is exemplified by Mikoto's actions during the episode. Furthemore, mooing sounds can be heard when Gekotas are focused in a scene.
It should be improbable for the car to flip and tumble several times in the air front first if the car wasn't running upon an incline, or Mikoto's Railgun to hit at a certain angle. The scene is less dramatic in the original manga.
Mikoto kicking the vending machine is reused animation from the 10th episode of the Toaru Majutsu no Index animation.
Two boys in the background pass by Uiharu and Saten with notable blushes after Saten flips up Uiharu's skirt.
After Uiharu sighs after having her skirt flipped up, you can see a man in the foreground walking to the right side. However, there is an animation error, as the man does not transition smoothly out of the scene and instead looks like he disappeared.

Episode 24 - Imaginary Number School District - Five-Element Agency

Although featured in the ending theme, this would mark the first time Accelerator is seen wearing his collar. This goes for Last Order as well, as she wears her dress that is featured in the opening for the first time.
Nearly all scenes shown in the epilogue are reanimated for the second season.
The Daihaseisai is first mentioned in this episode.
A promotional image of the anime adaptation of Toaru Kagaku no Railgun is tacked in at the end of the episode.
Several scenes of the previous episode, as well in episode 22, and episode 21 are used in flashbacks. However, scenes from Index's confrontation with the golem are reused and are later accompanied by the scene where Hyouka saves Index.
Index's "common sense" lecture to Touma regarding standard magic rituals back in Episode 21, is used as a flashback during Touma's final confrontation with Sherry.
During the scaffolding to scene, immediately after Touma tells Hyouka that her illusion is not over yet, and after they turn to look at Index arriving there, one can see that the color used to paint Touma's skin is not colored underneath his hair, making him look headless.

Overall Trivia
The identity of Komoe's friend who was researching into AIM diffusion fields is never revealed.
Note:
Since the story of Yamisaka Ouma was adapted for later in the second season, there is no indication that Touma broke into Academy City after returning.
This event does not occur in the anime adaptation, and can be assumed that Index peacefully returned to wait for Touma back in his dormitory, or followed Touma and Ouma to the outside.
Despite Touma actually forgetting to finish his homework in the anime adaptation, it is not brought up as he returns for school.
Since Toaru Majutsu no Index Light Novel Volume 02 was not adapted into the manga, Himegami Aisa is introduced for the first time during this story arc in the manga adaptation. And for reasons unknown, despite her powers not being sealed by the Walking Church, was tranferred from Kirigaoka Girls' Academy.
Sherry Cromwell did not use Automatic Writing in the anime adaptation, which can be assumed that she already knows who the Key of the Imaginary Number District is.
The food eating scene is completely removed from the anime adaptation. Moreover, all references to Hyouka's reasoning for being timid around Touma is missing.
Due to Himegami Aisa being introduced in this arc first, Komoe is the one who tries to contact Touma in the Underground Mall.
Due to Himegami Aisa not knowing Touma in the manga adaptation, Touma is contacted by Komoe instead for the first time.
In the original novel, a rifle bullet that were fired by the Anti-Skill, ricochets off the golem and hits Hyouka in the face. In both anime and manga adaptations, Hyouka is instead hit by the debris from the golem's attacks. Moreover, in the anime adaptation, Touma loses the Anti-Skill and is the only one who meets Sherry Crowell.
An important discussion between Touma and Komoe continues after this, regarding Komoe's unnamed university friend, who could see Kazakiri Hyouka as an important research material in the field of AIM diffusion fields. Both Touma and Komoe decide not to confide the truth about Kazakiri Hyouka to Komoe's friend. All adaptations remove this reference.
In both anime and manga adaptations, Kuroko and Mikoto does not take Index away, but has Mikoto come towards Touma alone, but is later teleported away by a jealous Kuroko, warning of Anti-Skill. Touma later leaves Index, who is looking at the wreckage of the golem, to go to Hyouka.
Episode 22 - Statue (Golem)

Mikoto has always been depicted as visibly taller than Kuroko, however, during the part where Kuroko decides to teleport both Index and Mikoto, she is shown to be just as tall as her.

Due to Yamisaka Ouma's story being transferred to the second season of Toaru Majutsu no Index, a continuity error exists in this episode, specifically, Touma's room remaining intact, though this event is not mentioned again in the novels as well.
When Index picks up Sphynx and rolls around the floor, she once hums A Certain Tune.
This is the first introduction of AIM in the series.
The mysterious Imaginary Number District is introduced.
Telesma is first mentioned in the series.
Aisa being a girl who goes unnoticed, would later become a recurring joke in every Toaru Majutsu no Index-related media she would appear in. This would be prominent in the Toaru Majutsu no Index-tan specials, poking fun on her apparent lack of notability.
This marks the very first time espers and magicians, if one does not take Tsuchimikado Motoharu into account, do battle with each other.
Komoe's car is the only traditionally rendered during its appearance.

Season 2
Epiosde 25 - August 31st (The Last Day)

This episode happened on the 31st of August in the Toaru Majutsu no Index timeline, as such events that happened after episode 18 are not referenced by the characters.
The tune that was first used in episode 15 by Index in the form of Aogami, is hummed by both Komoe and Aogami in two separate scenes in the beginning of the episode. Index hums the tune as well later in the episode.
ESP Cards are seen for the first time in the main storyline. ESP Cards were first mentioned in episode 10 of the first season and were first seen in the first episode of Toaru Kagaku no Railgun, being used during a System Scan.
Index references the Lotus Wand – Agnese's signature weapon – while watching Magical Powered Kanamin.
Touma and Index goes to the same restaurant that Accelerator and Last Order went to back in episode 19 of the previous season.
Ouma destroys Touma's room during this episode, though chronologically in episode 21 it is shown that his room is undamaged.
Interestingly, Mikoto hits a Cleaning bot because Touma ignored her, similar to that what occured in the first episode of the first season.
The Opening of the series, "No Buts!" shows Mikoto throwing electricity after she gets ignored by Touma but, if is seen clearly, the wood isn't an electricity conductor, but since it's not an isolant either, it's well possible to it transport energy.
Several scenes depict characters having a different shadows cast on their shoulders depending on their hairstyle, such as Touma having a spiky shadow on his shoulders, though sometimes they are completely changed into the type seen previously in the past season, though this maybe a standard practice by the studio.
As this occurs in the 31st of August, several scenes are reused from the previous season (specifically episode 18 since it occurred on the same day) to reference specific events.
The waitress on Julian Restaurant happens to be voiced by Kayano Ai, which is Itsuwa's voice actress.
Overall Trivia
The arc shows that both Touma and Accelerator enjoy the same coffee.
Note:
Although only described as an "esper power" in the original light novel, both manga and anime adaptations, uses two different powers for the esper that attacks Accelerator on the night of August 31. Unlike the novel however, their fates are ambigious.
The original novel had Touma show Mikoto the answers Mitsuki gave him, which, although coorect, foreshadowed the fact that the Mitsuki that was bothering Mikoto is not the same Mitsuki from the week before.
Only the novels reference the gaping absurdity of Touma leaving Etzali alone in the collapsed building. Here, he asks Tsuchimikado Motoharu to deal with him after their battle. This would also foreshadow the two of them working together as members of GROUP later in the series.
Ouma wrecking Touma's room is carried throughout all adaptations of the arc, and similarly, all adaptation makes no regard that it was wrecked in the next arc. The anime can be forgiven as Ouma's story is not adpated until the second season. This continuity error is apparent in the novels as well.
The original novel has Accelerator wake up on 11:35 AM before going back to sleep. All adaptations have Accelerator awake only once.
All adaptations had Accelerator flash back to his past where children attack him and then snowballed with the entire city trying to stop him. Both the anime adaptation and the original light novel made it vague; however, the manga adaptation has Accelerator get attacked by children due to the obvious instigation of the children themselves.
All adaptations play the scene differently. In the Toaru Majutsu no Index anime adaptation, due to time constraints, the first seasons completely removes Yamisaka Ouma's story, as such Accelerator does not see him nor Touma. However, during the second season, his story is adapted, and the only thing that is retained is Ouma bumping into Aisa, and it taking place much earlier in the afternoon, as Accelerator had arrived much later in the first season. However, it makes it look like that Accelerator notice no problems as he entered the restaurant or that both Touma and Accelerator used two different restaurants from the same branch. In the manga adaptation, only Aisa's appearance is removed.
There are no hints that the research center is the same as the </nowiki>7th Medical Research Center, the research center where the First Production Plan took place in Toaru Kagaku no Railgun.
All adaptations had no indication that the bullet that Ao used was an impact bullet, and can be presumed to be a normal bullet.
All adaptations do not reference the presence of ambulances in the distance just before Kikyou and Ao shoot each other.
Due to Ouma's story not being adapted until the second season, this scene is completely absent in the Toaru Majutsu no Index anime.
Episode 19 - The End (Last Order)

Episode 16 - Father (Tōya Kamijō)

This is the 2nd time it is demonstrated in the anime what would happen to espers using magic.
Index apparently shows no reaction when she meets Kanzaki in the form of Stiyl, even though she has kept her memories regarding him back in episode 6. But since none of the affected seemed to notice the change, it's possible Index didn't notice anything either.
Several scenes from the previous episode are used when Touma questions himself on why his father has involved himself in the occult.
When aired on TV, the opening PSI-Missing was used. This was changed for the DVD/BD release.
Episode 14 - Strongest vs. Weakest (Weakest vs. Strongest)
Major Event
Accelerator discovers how to create plasma using his powers.
With the aid of Mikoto and the clones, Touma is able to defeat Accelerator.
The Level 6 Shift experiment is suspended, and the clones are sent to facilities for readjustment on their bodies.

This episode confirms that Accelerator is physically weak due to him constantly relying on his powers.
Though Touma often gets into perverted situations unintentionally, this episode features his dismay at being unable to feel Misaka 10032's breasts.
At least 40 seconds of footage from the previous episode is reused in the beginning of the episode before actual original scenes for the episode are seen.
The 6th punch that Touma throws to Accelerator's face, shows a quick close-up. Here, there is a frame where one can see Touma's fist indentation remains on Accelerator's face for a short-while after he threw the punch. Also, note that Accelerator's face was slightly off-model during this quick scene.
Misaka 10032 should've been unconscious near the the railway tracks, as the previous episode shows. Though during this episode, her body is seen between two shipping containers and not any tracks, though thinking that Misaka 10032 simply moved towards that location is not that farfetched a speculation.
During the scene where the Misaka clones direct their electric power to the motor of the wind turbines, several scenes show that some of the wind turbines are close to each other. This should be impossible, as there is not enough room for the rotors of each wind turbine. The only plausible way the scenes would make sense is that the Misaka clones are directing their electric powers on the motor of a vertical-style wind turbine, even then, we have yet to see any examples of Academy City actually using vertical wind turbines instead of the horizontal ones. Also note, vertical wind turbines need at least a wide enough space for their rotors to move.

Episode 12 - Absolute Power (Level 6)

Mikoto uses her electric powers to attack Touma, who eventually succumbs to her, due to not using his ability to defend himself. This marks the first time Mikoto was able to hurt Touma with her powers since their meeting.

This episode features for the first time the infamous vending machine that eats money (later identified in Toaru Kagaku no Railgun as Vending Machine 7116), as well as Mikoto's infamous kick.
When Maika mentioned that she and Motoharu were making noise during the night, it may be a double entrede, considering how Motoharu is sis-con (someone who has sister complex or an attraction to sisters) and that Maika reads manga where siblings develop love for one anoter, which will be referred to later.
The episode mentions a school that does not focus on esper development.
Though unexplained in the anime, the celtic cross that Aisa has is a magical tool to prevent her powers, Deep Blood, from being detected by vampires.
This is the first time Aisa references Touma's incredible luck with girls he meet, which will expand tremendously in later episodes and novels.
The Misaka clone seen in the opening fires several bullets at Accelerator, however, only 1 seemed to have redirected back to the clone. Also, note there seems to have been little to no recoil at all during the firing of the rifle.
The Misaka Clone calculates her bullet trajectory according to the wind. Since Accelerator's redirection would send the bullet back following the bullet's original trajectory, the bullet should not have hit back directly at the Misaka Clone for there is also wind on the second trip.
Around the 10min 39sec time frame, Misaka puts the night vision goggles on but then has them in her hand in the next scene, and the back on her head in the next.
During the scene where Touma argues with Index and Aisa, you can see that Sphynx tail is wiggling back-and-forth, however, if one pays attention, you can also see Misaka 10032's eyes moving back-and-forth as well watching Sphynx's tail wiggling.
Just before Misaka shoots to Accelerator two people that resemble to Akito Takagi and Moritaka Mashiro (Bakuman protagonist) walks near him.
Episode 9 - Vampire Killer (Deep Blood)
Major Event
Touma defeats Aureolus, saving Aisa and Index and thus completing Stiyl's mission in Academy City. Aureolus is changed to a different person after his defeat.
Misawa Cram School is closed down after the confrontation.
Aisa temporarily moves in with Touma and Index.

This is the first time that someone has managed to completely cut-off Touma's Imagine Breaker from his body.
Aisa is the second girl to move in with Touma, though it is short-lived.

Overall Trivia
This arc does not appear in the manga version, and Aisa is introduced instead during the Kazakiri Hyouka Arc, where she enters A Certain Highschool as a transfer student and warns Touma about Kazakiri Hyouka. Stiyl Magnus is also later reintroduced to Touma much later during the Orsola Aquinas Rescue Arc.
According to Touma this arc occurs two weeks after he lost his memories which was July 28, this makes the date of this arc occur on 8 the 8th of August. In the Toaru Kagaku Railgun anime, Aisa is shown to have already acquired the cross necklace from Necessarius, which would imply that this arc has already passed. However, it is later revealed that the final date of the Poltergeist Arc is August 9, a day before the beginning of Sisters Arc in the Toaru Kagaku no Railgun manga timeline. We can infer that the there are timeline discrepancies in the Toaru Kagaku no Railgun anime adaptation.
The frog faced doctor points out in both the anime and the novels that in 10 days this has been Touma's second hospitalization, with that is possible to locate this arc in either the day 6 or 7 of August or perhaps even slightly earlier, that would mean that this arc does not conflicting with the others introduced in the Railgun anime.
Note
Despite the anime adaptation's efforts to keep true with the novels, there are still several glaring liberties taken by the adaptation. One of the most egregious examples is the Deep Blood arc, taking away a considerable amount of content from the novels for the adaptation. Below is a list of notable examples:
Important:
Tsuchimikado Motoharu appears alongside Aogami in this arc, similar to the previous arc as the anime introduced him early. Although, he would often be referenced in-name only up until his actual appearance in the 4th Toaru Majutsu no Index Light Novel.
With Tsuchimikado Motoharu present in the arc, he is instead used for the rock-paper-scissors scene instead of Index.
The anime adaptation skips the reason for his deployment in saving Aisa, as well as her power's apparent importance to the magic side. It also moves Aisa's backstory to the cold opening at the beginning of episode 7 of the first season.
The anime adaptation also introduces Aureolus name as well as some minor backstory earlier. Furthermore, the adaptation also reveals Aisa being the bearer of Deep Blood much later.
Touma and Stiyl's reaction is much more intense in the original light novels, as Touma realizes that the knight is still barely alive and tries to feebly save him. Here, Stiyl gives the knight its final rites before finally dying.
The anime failed to note why Stiyl and Touma went to look at the cafeteria in the first place. In the original novel, the reason they went to the cafeteria was because of the fact it was nearest to a secret room in the building that they were searching for.
The entire Aureolus Dummy subplot is completely removed in the anime adaptation. Since it doesn't appear in the anime, both Stiyl and Touma confront the real Aureolus Izzard earlier.
Since the entire Aureolus Dummy subplot was cut, the anime connects the previous scene with Aisa with the scene following Touma defeating the Aureolus Dummy. Other changes were necessary for this to work, such as the girl that Aisa gave first aid to remains alive.
Since the anime adaptation cut out the part where Aureolus mentions to Touma on how he has Index, Sphynx is used instead in an attempt for Touma to realizes later that Index is in Misawa Cram School
Also, since the Aureolus Dummy does not appear in the anime, Index does not witness it's horrific death at the hands of Aureolus and is unable to recognize Aureolus' Ars Magna.
The anime failed to note on how no one other than Touma and Stiyl seemed to realize that an entire building was razed by the knights is because of the fact that something similar to Stiyl's Opila runes were applied to around the school.
The anime made it completely vague as to how Touma were able to figure out were Aureolus office, as it did not indicate at all that the office was in the north block. Furtheremore, their conversation on how Aureolus was Index's partner and wanted to save her did not occur until much later.
The anime features this scene a little less cruelly for Aureolus. Here, Index does not regain consciousness but only calls out to Touma's during her sleep. Furthermore, Aureolus does not try to kill Index as a result of that.
Touma's injuries as a result of the bullets fired by Aureolus, is downplayed in the anime adaptation.
Since the anime left out the part where Stiyl scatters his runes upon being ripped to shreds by Aureolus, this entire conversation regarding Stiyl actually helping Touma during his armless rampage does not occur. However, instead in the anime adaptation, Touma is seemingly able to use his Imagine Breaker (with its distinct sound effect) without the use of his right arm, which may bring up plot holes whenever the subject of Touma's right arm being removed is brought up again.
Indeed, the subject regarding the removal of Touma's right arm and the secrets of the Imagine Breaker will not appear again until 20 volumes later, during World War III.
Others:
Stiyl comments in the original novels that although Aleister's not telling him of the Roman Catholic Church Knights working in the city is understandable, since Science and Magic are different sides after all, and if the Magic side loses capable fighters it would be to Aleister's advantage.
The adaptation failed to cite that Touma blames himself for not saving Aisa back when he met her in the restaurant. Believing that she had escaped the school and that she was looking for money to get away, hence his guilt for not giving her money she asked for. Touma also mistakes Aisa leaving with the cram teachers without trouble as a sign of her not wanting anyone getting involved.
This would mark the first time Touma did not have the intention of simply beating an opponent but seriously injuring them. Here, Touma actually had the intent of killing the dummy.
This intention towards opponents would not appear again up until the 17th novel, where Touma faces off against terrorists in an airliner.
The anime switches the scene with the knights using Gregorian chant and Touma dispelling Stiyl's memory loss, doing the former first and the latter second. In the anime adaptation, both Stiyl and Touma are outside of the Misawa Cram School after they had their memories erased, and were there to witness the bombardment of the building as well as reconstruction.
Also, since the anime completely removed Aureolus saying to Touma that Index is in the building, Sphynx is used instead as the indicator for both Touma and Stiyl.
The narration states the possibility that Aureolus may not have come up a ridiculous imaginings of dragon head on his own. Furthermore, the narration mirrors the previous question opened up by Aleister Crowley in the beginning of the volume, regarding what existence Imagine Breaker proves if Deep Blood proves the existence of vampires.
Episode 8 - Gold Transmutation (Ars Magna)
Major Event
Touma meets Aureolus and does battle with him, though they are interrupted by Aisa.
Aureolus abducts Index.
The Thirteen Knights of the Roman Catholic Church is completely defeated by Aureolus.

This episode introduces the Roman Catholic Church as a religious faction as well as the 13 Knights of the Roman Catholic Church in the Toaru Majutsu no Index universe. Though not implied in this episode, the defeat of the knights as well as Aureolus Izzard's reversal of the Gregorio's Chant, severely weakened the Roman Catholic Church's power.
This episode features espers using magic for the first time, and showing the dangerous effects it causes to the esper.
Aureolus Dummy, who appears in the novel when Stiyl disables the control mechanism of the Greogio's Chant Replica, was cut down in the anime.
The glass breaking animation as well as its accompanying sound that is played when the second half of the anime starts, is played during Touma's conversation with Index on the phone.
A different sound is played instead of the sound that plays whenever Touma dispels something, when Touma touched his head after Aureolus wiped his memory.
When Touma tried to punch Stiyl in the playground, he runs for a few seconds towards him. However, it is shown that there is only a short distance between them, and there is no need for Touma to actually run as there is no room.
Deep Blood Arc
Episode 7 - Misawa Cram School (Science Cult)

Touma and Index meet Aogami and Motoharu, with Touma meeting them for the first time since losing his memories.

Episode 6 - Fantasy Killer (Imagine Breaker)

Touma with the assistance of Stiyl and Kanzaki, manages to stop John's Pen Mode Index's rampage, at the cost of Touma losing his memories.

Necessarius allows Index to stay with Touma.
Touma meets Index again after losing his memories, with Touma lying to Index about losing his memories.

During this episode it is shown that Stiyl can summon Innocentius without performing the incanation that was shown in episode 02, though the conditions for this is unknown.
This is the first instance that Touma says his "Destroy/break your illusion." phrase.
As of this episode, Index is still the only person to have ever confessed to Touma.
The Tree Diagram was destroyed a few minutes after midnight on July 28. According to the news reports around Academy City, it has been 2 weeks since the interruption in communication with the Orihime-01 sattelite - which carried the Tree Diagram. This would mean that it is at least early August, around day 8 or 9 of the month. In the Toaru Kagaku no Railgun anime adaptation, it was shown that the Poltergeist Arc ended on August 9, which would indicate that the Sisters Arc will occur the very next day; however, it was shown in the Toaru Kagaku no Railgun anime that Touma has already met Himegami Aisa prior to August the 9th in the anime, in this episode, Touma has yet to be relieved from the hospital - much less meet Aisa. This would mean that there are slight discrepancies between the Toaru Kagaku no Railgun and Toaru Majutsu no Index anime timelines.
During the broadcast reporting about the lost of communication with the Orihime-01 sattelite, Misaka pasts by the spot where she and Touma last met before his memory loss.
Even though the distance between Index and Touma is not longer than a few feet, it took Touma an unusual amount of time to come into contact Index when he charged towards her.
When the sattelite was destroyed by Index, it produced a sound, which should be impossible since sound cannot travel in space.
Overall Trivia
In the Toaru Kagaku no Railgun manga, the blackout caused by Mikoto due to fighting Touma on a bridge after failing to get information on the Level Upper is the same event that appears in the beginning of Toaru Majutsu no Index. This event was completely changed in the anime adaptation as Mikoto caused the blackout while fighting Elder Sister, the anime still indicates that Mikoto fought Touma on a bridge which implies that in the anime's timeline, there were two blackouts that were caused by Mikoto.
Komoe later references the incident where the roof of her house got destroyed, in episode 17 of the Toaru Kagaku no Railgun anime to Tessou Tsuzuri.
Heaven Canceller not reporting Index for being an outsider shows his professional neutrality as a doctor, a trait which he carries throughout the entire series until the end of Academy City Invasion Arc, where he tells Aleister Crowley to stop his playing with his patients, though he remains neutral to the activities of the characters even after that.
Index is still the only girl in the series to have confessed to Touma of her feelings.
The anime adaptation combines the end of the arc with the beginning of the next one, more specifically the prologue of the 2nd Light Novel.
Note
Narration states that they have crossed each others' paths sporadically for nearly a month. This is expanded in Toaru Kagaku no Railgun, where they met for the first time in early June of the contemporary year.
There is a differing account on how Touma dealt with Mikoto's attack. In the anime adaptation, the Railgun simply passes Touma by, however, in the Toaru Kagaku no Railgun interpretation of the scene, Touma easily blocks the Railgun. In the original novel it is told by Touma that he had to defend himself from a barrage of attacks such as Iron Sand and the Railgun.
According to Touma's hypothesis, Index may have been able to infiltrate the city due to the power outage that occured in July 19 knocking out the surveillance satellites of the city.
In the original novels Index explains on how she ended up in his balcony because she somehow slipped while moving from rooftop-to-rooftop, because she was trying to escape from people who were after her. She also explains to Touma on how she survived because she was wearing her Walking Church.
The manga version follows the novel more closely as well, though she mentions the Walking Church only later.
The scene is extended in the original light novels and Index and Railgun manga, where Touma laments that he had to meet a magician and Mikoto on the same day.
Furtheremore, both the original novel and Index manga adaptation expands on Touma's fight with Mikoto on the bridge before Touma escapes.
Both novel and Index manga adaptation features Stiyl switching the rune marks written on paper to the his now-trademark Rune cards.
The original novels implies Stiyl is still trying to secure Index while Kanzaki talks with Touma.
Interestingly, in the Light Novel Touma's determination shown in his words and actions shook Kankazi's mind to the point she was blown away and her nodachi flew out of her hands by Touma's fist, when he was injured to the point where his own fist would bleed profusely and cannot possbily physically harm anyone. This in later novels would fuel speculations and rumors in the Magic Side that Touma have enough power to match a Saint in combat.
The entire scene is played differently in the manga adaptation. Both Stiyl and Kanzaki do concede, however, they do not leave Index's side as she is overcome by her deteriorating health. Here, Touma leaves them to try and find a way to save Index his own way without erasing her memories.
The scene plays completely differently in the manga adaptation. There Touma has already asked Komoe regarding memories after leaving Kanzaki and Stiyl (this where he Stiyl challenges Touma to remove the magic of the cross that will help Index). Also, Index does not awaken completely from sleeping before Touma tries to negate the collar.
Both the manga and light novel doesn't specifically show that the Tree Diagram was the satellite that have been destroyed by Index's Dragon's Breath. However, this is later confirmed by episode 6 of the anime adaptation an later ensuing corroborating evidence.
Index does not read the letter in Index manga adaptation, and is instead read by Touma before being visited by Index in his ward. It is also not shown that the letter exploded for Touma.
Episode 5 - 12 o'clock (Limit)

Episode 4 - Perfect Memory Ability

Touma does battle with Kanzaki and is completely and utterly defeated by her.

The song Ippou Tsuukou, Accelerator's leitmotif, is played during Kanzaki's explanation regarding Index's perfect memory, despite Accelerator not appearing for another 6 episodes.
Kanzaki's sword is called Shichiten Shichitou, which literally means "Seven Heaven Seven Swords"; while her attack is called Nanasen (七閃), which literally means "Seven Flashes", which according to Kanzaki, can kill a person seven times in a split second. Also according to Kanzaki, what awaits someone after going through Nanasen is Yuisen, which literally means "The Only Ray", though it is unknown what this implies.
When Touma is first kicked on to the ground by Kanzaki, his injuries disappear.
The scene where grimoires are spinning around Index is reused in this episode, except Touma and Kanzaki are the characters instead of Index.
